A DOT physical at FICOM is $105, and most drivers are done in about 20 minutes

You do not need an appointment. You can walk in Monday through Friday between 7:30 am and 4:00 pm, and a certified DOT Medical Examiner will complete your exam. Most drivers are in and out in about 20 minutes. Emergencies take priority and some days are busier than others, so if you are on a tight schedule, midweek mornings are your best bet.

What to bring

Your health history and medications drive most DOT requirements, and bringing the right paperwork the first time is the difference between one visit and two. Bring the items on this list that apply to you.

A valid driver’s license.
A complete list of your medications.
Your glasses, if your license requires corrective lenses.
Your sleep study report, if you have had one and it was negative for sleep apnea.
A CPAP compliance report, if you wear or are prescribed CPAP therapy.
An insulin form, if your medications include insulin.
A medication clearance letter for commercial driving, if you take any mental health medications. This is an FMCSA requirement.
Your most recent stress test or echo and a cardiology clearance letter, if you have a history of heart attack, bypass surgery, or cardiac stents.

Your interstate or intrastate designation is up to you, not your examiner

Drivers sometimes come in believing their medical examiner sets their interstate or intrastate designation. That designation is a self-certification you make with the state through the DOR and DMV, and your examiner cannot change it for you. If you are 21 or older, you will automatically receive a federal certification that allows you to drive nationally.

How much is a DOT physical?

A cash-pay DOT physical is $105. Payment is due before check-in, and we accept cash or card. A 6% fee applies to card payments. If your employer is paying, they need to send us written authorization before your visit, and the Employers page explains how that works.

Do I need an appointment?

No. DOT physicals are walk-in, Monday through Friday from 7:30 am to 4:00 pm.

I was told to move from accepted intrastate to non-accepted. Does my examiner do that?

No. That designation is a self-certification through the DOR and DMV, not something your medical examiner sets. The two links above will help you determine your category and update your status.
